THE FIRST PRACTICE IS AUGUST 1, 2011
 
Here, you will find the necessary forms and information to get your child on the playing field. In order to participate in the HFFCA or any Pop Warner program, you will need to submit the following completed forms:
- The 2011 Participant Contract and Parental Consent Form (Registration form, download form below).
- The Pop Warner Physical Fitness & Medical History Form (Download form below).
- Participant's Current Year Final School Report Card
- An Original Birth Certificate (new players only)

REGISTRATION DATES, TIMES & LOCATIONS
We will also be accepting registrations at the Eli Whitney football practice field (located on Arch Street between Pine Rock & Fairview Avenues) Monday through Friday from 5:00pm-7:30pm during the month of August once practice begins.

***Participant Ages 5 - 15 years***
We Accept Cash, Money Orders, Checks & Credit Cards.

SEND YOUR MAIL-IN REGISTRATIONS & PAYMENT TO:

- Flag Football & Tiny Mite Cheer - $75.00
- All Other Levels - $195 for the First Child, $185 for the Second Child & $100 for Each Additional Child.
When downloading the above forms, there may be a delay depending on your computer and internet provider's download speed. Please be patient. These forms are mandatory and must be submitted before the participant may begin any practice. This is a Pop Warner rule.
Please be sure to read and complete the registration form properly. This form contains important information that is applicable to both the parent and the participant.
The physical/medical form provided in the link above is the only form that will be accepted. Please be sure your doctor's office uses this form to avoid delays in registration.
The only form that will be returned to you is your child's original birth certificate. Please make copies of any other forms you wish to keep before submitting them to us.
